We describe a case of pancreatic tumor associated with a giant type IV hiatal hernia that had prolapsed into the posterior mediastinum. We initially performed hiatal hernia repair, followed by laparoscopic distal pancreatectomy, because the hernia repair enables performing pancreatectomy safety and in the normal anatomical position.

Infection of urachal remnant may cause recurrent abscesses. In the current case report, we describe an urachal remnant infection leading to a retroperitoneal abscess, which is extremely rare condition. In such cases, the recommended treatment is urachal remnant resection.
Rare vascular anomaly of the common hepatic artery in gastrectomy

Aberrant common hepatic artery originating from the left gastric artery without connecting the gastroduodenal artery is extremely rare. In gastrectomy, accidental ligation or injury of the aberrant common hepatic artery can lead to hepatic arterial ischemia. Reconstructions of CT images may be useful to avoid complications related to vascular anomaly.
